Is Fade to Black actually missing or just cut? On Jake's site, he has Fade listed as cut, not missing.

I love the background and that green/yellow color it works perfect with the color of the background. Now, I don't wanna be picky, but I think you should write Ottawa, Canada and not Ontario, cause Ontario it's the province's name and Ontario includes Toronto, Ottawa, Hamilton...big cities like thoses. Just a comment

Not really any different than writing "San Francisco, California, USA" though, is it? I could change it, its just in my mind I break down things for organization, which is one reason why I write my dates DD-MM-YYYY, from least to greatest.

But by saying "San Francisco California, USA" we know exactly that is in San Francisco. But Ontario, Canada could be anywhere in that big province that has important cities such as the ones I mentioned and also, in those cities Metallica has played before. I am not saying that is wrong, but it could lead to some confusion.

Yeah, the cover is prefectly fine. But I would change the file name to 1991-11-18_OttawaCanada and then change the title you gave the cover/stomp too.
